TB HIV Care (THC), an established South African non-profit working in HIV and tuberculosis prevention and care since 1929, is recruiting three Case Officers / Enrolled Nurses to support its Counselling & Testing department in the Eastern Cape. The positions are based at Tembelihle Clinic, Philani Clinic (Emalahleni) and Gardens Clinic, serving communities in Cala, Ladyfrere and Queenstown. Purpose and role
The primary purpose of these roles is to implement and monitor the Case Management Model (CMM) to strengthen adherence support for people recently initiated on antiretroviral therapy (ART) until they are ready to decant into differentiated models of care. Case Officers will play a frontline role in ensuring clients on ART receive timely follow-up, linkage to community resources, and psychosocial support designed to improve retention and viral suppression.
Core responsibilities
Daily tasks include capturing client details — especially those on ART for less than a year and newly initiated patients — onto the CMM spreadsheet and tracking upcoming appointments. The role extends to managing records for clients with high viral loads, pediatric patients, pre-ART clients and pregnant women. Case Officers will make proactive telephone contacts and coordinate daily linkage to community health workers or other community resources for clients who miss appointments. Home visits are required for patients who miss appointments, are not virally suppressed, or need additional care. Routine SMS appointment reminders, professional responses to client messages, and referrals to support groups form part of the patient-centred approach.
Essential qualifications and experience
Applicants must hold a Grade 12 certificate; an enrolled nurse qualification is an advantage. Candidates should have at least two years’ relevant experience in counselling and testing, and be competent in adherence counselling, basic HIV testing and counselling, and index case tracing. The ability to communicate in English is required, and isiXhosa is an advantage given the local context. A clear criminal record is also required.

Networking and knowledge-management abilities will help integrate clinic-based care with community services.
