Job Description
As a Community Health Worker at Anova, you will provide essential health services and support to individuals in their homes. Your role will involve a variety of duties focused on health promotion, prevention, rehabilitation, and palliation.
Key Duties and Responsibilities
- Physical Care:
- Assist patients with personal hygiene and daily living activities.
- Provide wound care and stoma therapy.
- Monitor vital signs and assist in emergencies.
- Support patient mobility and exercise.
- Care of Patient’s Environment:
- Maintain a clean and safe living environment for patients.
- Manage infection control measures.
- Health Education:
- Educate patients and families on home care practices and available resources.
- Provide nutritional guidance and meal planning.
- Referral and Reporting:
- Refer patients to health facilities when necessary.
- Report any changes in patient conditions to supervisors.
- Administrative Duties:
- Maintain accurate patient records and documentation.
- Participate in training and organizational meetings.
- Medication Management:
- Supervise and administer medications as prescribed.
- Ensure patients adhere to their treatment plans.
Minimum Qualifications and Skills Required
- Grade 10-12
- Home-based carer qualification/certificate
- Experience working within communities
- 10-day HIV Counselling Certificate (advantageous)
- TB/HIV Adherence Counselling Certificate (advantageous)
- Knowledge of HAST & NCD
